Our office is closed as of March 1st, 2026. We are so grateful to our patients, staff, and the North Portland community for the trust and support you have given us since 2015! 

For former patients, here are some tips on how to get assistance during your transition to another dental provider.

• Requesting Records: We can send your digital records to you or to another dental office by email. To request a records transfer, please contact us by phone, text, or email and let us know where you would like to have your records sent. If you are scheduled at another office, please include the date of your first visit so we can prioritize the transfer.

• Paper Charts: If you were a patient prior to 2013, we may have a physical chart in addition to your digital records. Let us know if you’d like to pick yours up.

• Dental Models: If you’ve had implants, nightguards, or orthodontics, we may have your tooth models. Please reach out if you’d like to keep them.

• Treatment Questions: If you have concerns regarding recent or unfinished treatment, please reach out via phone, text, or email. We will continue to monitor messages as long as necessary.
